Consent form as electronic form in Nettskjema

1. Go to nettskjema.no. Log in with your UiO user name and password.
2. Click "My forms"

3. Click the "Shared with me"tab.

4. Scroll down to "Forms I can copy". Find the form "Consent for recording of sound and/or video", and click the "Copy" button next to the form name.

5. The web form is then created on your user and you can change and distribute is as if you made it yourself. If you are unsure of how to do this, please see Help for Nettskjema. 
   Remember to make changes to the web form before it is distributed, so that the content reflects the admission situation / subject / contact person.
